Just like the physical part of personal training, a diet plan very depending on client’s fitness goals, age and weight. For example, if a middle aged man is looking to lose a significant amount of weight, he would need to do a few things. First, in order to lose weight your body need to burn carbohydrates that have been consumed in the past twenty-four hours. Once your body has depleted all unused carbohydrates, it can now access your bodys stored fat source and start to burn it off. But in order to burn carbohydrates in the first place, he must engage in high intense exercises, which min in their late thirties usually don’t do. So what needs to happen is he must alter his diet to eat six times a day. The content of his diet would be low carbs and proteins. The reason for him to eat six times a day is to keep his metabolism running so he can burn fat during his daily routine. Along with a diet, comes exercise with his personal trainer. His workouts would consist of very low weight, with a high number of repetition. The reason for low weight workouts is because he would be low on carbs which would highly effect his strength. The high amount of reps will cause his muscles to fire quicker, causing him to burn carbohydrates then start burning stored tat ( Trainers can work in many different places. For example, if a trainer has a group workout in urban areas, he could meet his clients at a local park or running trail. The most common place would be the gym, where a personal trainer can train one or more individuals. Trainers even go to clients homes for personal workouts. That’s usually the case for elderly people who would have a hard time leaving their home. Qualified trainers travel to hospitals to train rehabilitated patients who may have been injured in an accident and is in need to re-train their muscles (United). To become a trainer is not very difficult. A degree is not needed, but would be very beneficial. My personal trainer, Brodie Priestley recommended a degree in kinesiology and nutrition. Also having a background in sports and powerlifting benefit me. Even though a degree in not required, sources indicate it would do nothing but help (United States). During my interview I asked my trainer Priestley what made him choose kinesiology as his profession. â€Å"It was simple† Priestley replied. â€Å"My father was a world class power lifter, benching over five-hundred pounds as a senior in high school. So like every kid, I wanted to be Just like my father. I fell in love with lifting weights and lay sports. The more I lifted weights, the more results I saw and I began to feel better about myself,† Priestley explained. â€Å"l quickly got addicted to lifting weights and wanted to keep progressing. I began to educate myself more on lifting weights and ways to get bigger and stronger. My obsession with lifting weights and muscles development got to the point where I stopped playing sports to focus on powerlifting. † Priestleys hard work was paying off as he took first place in numerous powerlifting events. â€Å"l wasn’t Just winning, I was blowing my competition out of the water,† Priestley explained. Priestley worked his way up to become the number three power lifter in the nation, and later broke the world record in bench press for his weight class. It was obvious that I was good at what I did, so I wanted to help others accomplish their physical fitness goals. So I began Personal training at a local gym. I quickly fell in love with helping others and seeing them progress. It’s a cool feeling, I couldn’t be happier with what I’m doing† Priestley explained to me. But I guess the hardest part of being a personal trainer would be getting some clients to trust you and to buy into your system. † Priestley continued on saying that some people who may have been out of shape most of their life have a hard time believing that they can lose weight and get in shape by following my weight loss program, which makes it difficult to train that individual. Next I asked Priestley what he liked most about his Job. Priestley responded by saying â€Å"The best part of my Job is seeing true results in my clients. There is no better feeling as a trainer knowing that you helped someone get healthy and physically fit. In most cases that is a life changing event for people who used to be highly over weight and unhealthy. That person can now walk around proud of their appearance and have all the confidence in the world, all because of my help. To me there is no better teeling. † Even though I learned that an education is not need to become a trainer, I asked Priestley what steps he took in school to get where he is today. After all, doesnt the United States have the most heavily armed population on the earth? Are we not the worlds most violent people? Surely these facts must be at least casually connected. Therefore the apparently desperate need to do something about the vast quantity of firearms and firearms abuse is obvious. Guns are employed in an enormous number of crimes in this country. In other countries with stricter gun laws, gun crimes are rare. Many of the firearms involved in crime are cheap handguns, so-called Saturday Night Specials for which there is no legitimate use or need. The public is polarized on the issue of gun control, Anti-gun control activists believe that it is each and every Americans individual right to bear arms. After all, the Second Amendment to the Constitution states that: A well regulated Militia, being necessary to the security of a free State, the right of the people to keep and bear Arms, shall not be infringed. Advocates of gun control say that even with 20,000 gun control laws already in existence, the serious problems due to firearm misuse continue. Obviously, the controls that have been designed have not been sufficiently effective. Therefore the pro-gun controllers argue, we need more uniform legislation, more extensive gun controls, and effective enforcement. Various pro-gun control organizations disagree on methods of gun control needed. For example, there are individuals who would ban all handguns as well as those who take a less radical stand and who would simply increase the controls on firearms. The moderate gun control groups propose measures such as requiring an individual to successfully complete a firearms safety course before being allowed to possess a gun, or to wait for a mandatory period of time before taking possession of a gun. Today, there are approximately 20,000 different gun control laws in existence, ranging from those enacted by municipalities and states, to those enacted by the federal government. Gun control is ineffective and tougher sentencing of criminals and stricter parole policies would do far more to combat crime. Statistics show that Canada is less violent than the United States. Fewer guns are only part of the story. The inner-city slums of the United States are murderous, bombed-out-looking places. American visitors to Canadas big cities often ask where the slums are. The answer is that there really arent any slums, and the lack of violence there reflects it. Canadas more generous welfare benefits and universal health insurance have made for safer cities. The contrasts between extreme wealth and extreme poverty are fewer and less striking. Poor inner-city families do not disintegrate to the extent they do in black American ghettos. Canadian murder rates in big cities are about the same as in isolated rural areas. According to THE ECONOMIST magazine; Blacks, 12% of the United States population, account for 48% of murders, mostly when inner-city blacks kill each other. (The Economist July 10-16,1993, pg 38) Few of these guns if any are purchased from retail gun stores. Gun laws will not keep guns out of these ghettos. The founding fathers included Second Amendment to the Constitution because they were very aware of the fact that there might once again come a time when American Citizens would have to fight for their freedom. Patricia Lee of Balch Springs, Texas was running for the Texas House of Representatives in 1992 when she wrote the following about gun rights. I watched while the rebel managed to extricate itself from crowd and made his way to a smaller gate that no one else had seen in their rush to follow the everyone else. The situation sparked the memory of an essay once read: Self Reliance by Ralph Waldo Emerson. It reminded me specifically of Emersons description of the ship that must tack in order to finish a course that is set against the wind. Emerson puts forth the idea that not only must man rely upon himself, but cut out his own path in the world and set himself apart from it. He should be like the man that made his tracks visible by steering them in a different direction from that of all the others of the crowd. Rather than do what is dictated by society, people should act according to the desires and impulses of their own nature. In Self Reliance Emerson writes of the rare quality in men to resist the pressures of their society to conform. This pressure often comes about in the form of accepted behaviors, the learning of which is termed maturity. Yet Emerson goes against this when he says that whoso would be a man, must be a non-conformist (Emerson, 261).This offers an opposite view of maturity which states that the ability of persons to know intimately their own nature and to respond to their impulses is a truer sign of maturity than familiarity with and obedience to societys norms. He suggests that the act of exploring ones own psyche offers more rewards than that of learning and performing societys proper duties for two reasons: The first will go directly to the exact needs of the human inpidual, while the second will only hinder and hide the inpiduality that leads to creativity and progress for man. One must remember that Our only access to truth, goodness, or to life itself, is through our own understanding and our own judgments (Miller, 2006). This lets us know that the inpidual is the only one who can tell for sure what he or she is to be. Trying to keep tradition alive, according to Emerson, does something that is similar to stunting the development of mankind. Yet it is surprisingly easy to do. What is difficult is to rise above the collective will express ones inpiduality. However, Emerson writes that the accomplishment of such a feat may serve for the whole distinction between greatness and meanness (Emerson, 263). The difficulty in rising to the level at which one might be called an inpidual is hampered by those in society who pretend to know what is the purpose of all other human beings. Yet to abandon society and go into solitude is a way in which a person can shut out the droning of the crowd and find a place where nature can be free to influence the will. Emersons concern is that peoples actions indicate their character, and when a mans actions are dictated by the traditions of decades or centuries, a third party will have trouble detecting the true character of the man that performs them. Such a man is indistinguishable from the other conformists that surround him. He no longer sees with eyes cleansed of the effects of the group mind and institutional constrictions (Kateb, 1995). Such persons have lost the carefree attitude of the youths who (unlike adults) are without self-consciousness and do not seek to flatter or pacify persons; for the young person, no one is set up on a pedestal. In keeping with this, Emerson describes the character of youth that gives the kind of self reliance that he praises. He cumbers himself never about consequences, about interests: he gives an independent, genuine verdict. You must court him: he does not court you (Emerson, 261). No power that a person thinks he can see in any human would make the youth and the self reliant man to feel the need to give false praise. Emerson highlights his belief by his idea that books and scholars are to be treasured for their ability to inspire the inpidual to greatness (Goodman, 2005). He identifies the way men idolize such persons that distinguish themselves through self reliance as an example of something contradictory. Men are equally likely to revere the ideas given by persons in books as they are to put kings and leaders on pedestals. Yet, Emersons reaction to this is negative. He writes: Our reading is mendicant and sycophantic. In history, our imagination plays us false. Kingdom and lordship, power and estate, are a gaudier vocabulary than private John and Edward in a small house and common day's work; but the things of life are the same to both; the sum total of both is the same. Why all this deference to Alfred, and Scanderbeg, and Gustavus? Suppose they were virtuous; did they wear out virtue? As great a stake depends on your private act to-day, as followed their public and renowned steps. When private men shall act with original views, the lustre will be transferred from the actions of kings to those of gentlemen (Emerson, 268). In this passage, Emerson reminds the reader that the life of the private and unknown man is as worthy of praise as that of the most powerful king, once that life is lived in truth and keeping with the nature at the core of the persons being (Beran, 2004). Another point Emerson makes is that the praise that persons give to history and its events is directed in the wrong way. He speaks of this largely as it regards tradition and convention. Yet, he further causes the idea to include the private actions of even the inpidual himself. He believes that man should not cling even to his own actions and beliefs of the past merely because he has kept them for months, or even years. Movement is involved in living, and the growing and changing man cannot mature unless he goes back and reassesses his beliefs and actions from time to time. It is no crime or shame to change ones mond if one has found a reason to do soone that more accurately expresses his or her nature. To rely only on ones memory of what one has said or done in the past in order to avoid public contradiction might lead to an even worse contradictionthat of oneself as one has now evolved to become. Emerson gives the metaphor of memory as a corpse that one drags about. A corpse is dead, while a persons character is a living, changing thing. Emerson continues: a foolish inconsistency is the hobgoblin of little minds, and in the presence such faulty adherence to consistency, a great soul has simply nothing to do since all would have already been done (Emerson, 265). According to Emersons essay Self Reliance, men, like that lone man in the crowd, must be willing to go against the pressures of conformity and be true, not to tradition, but to themselves. Only in such an environment is greatness thought of and nourished. A person cannot know precisely who he is if he continues to be ruled by societyand the inpidual, if he can be identified, is more likely to contribute something worthwhile to society when s/he is truly being him/herself. Emerson began his essay by identifying a truly original piece of writing. Had the writer been a conformist, his nature (which differs from that of all other men) would never have been revealed, and his originality would have been lost in the dullness of everyday life. Though the choice to be oneself includes the choice to disregard all the points made in the essay, yet only that choice matters. Abstract: This paper hypothesises that, whilst Six Sigma as a change and improvement strategy is delivering significant business benefit to practitioner organisations, it has not been successfully adapted to deliver similar benefits across supply chains. It demonstrates by reference to the literature that most published applications of Six Sigma in supply chains are related to the application of traditional internal Six Sigma methodologies to the internal processes of a supplier to the â€Å"Six Sigma Organisation†. In this paper, the issues particular to an application of Six Sigma in a broader supply chain context are discussed, with reference to specific supply chain issues. It is concluded that Six Sigma does have something novel to offer organisations over and above the contribution of existing approaches to supply chain improvement, and a conceptual model is proposed that is consistent with the literature and has potential to support such an introduction. Although rooted in the supply chain realm, SCOR adherents see a role for the methodology as the gatekeeper – identifying the projects most likely to render ROI using SCOR, Lean or Six Sigma. There is already a natural link between Lean and Six Sigma at the program and project execution level. The model integrates the Balanced Scorecard, SCOR model (Supply Chain Reference model) and Six Sigma DMAIC (define, measure, analyse and improve) methodology in a two-level framework. This is a strategic-level cycle, developing focused projects to generate maximum business benefit, and an operational-level cycle, applying Six Sigma and lean tools in a DMAIC cycle to deliver supply chain improvements. Cautions and requirements for the success in practice of such a model are discussed and it is concluded that the model should be tested in practice to validate and develop further the methodology. Keywords: Six Sigma; Supply chain improvement; Lean; SCOR model; Variability reduction 1. 0INTRODUCTION 1. 1 General Introduction Six Sigma Process Improvement is a rigorous approach to improving business processes by addressing the underlying causes of variation that lead to poor performance as experienced by the ‘customer’, who is the recipient of the outputs. The early exponents were Motorola and GE in the 1980s. Since then, many organisations ranging from manufacturing to service in all sectors, have successfully deployed Six Sigma to deliver measurable cost, quality and time based improvements. 2. 0 LEAN SIX SIGMA In the past, Lean and Six Sigma have at times been viewed almost as rival methodologies, with some companies choosing one or the other as their primary improvement vehicle. Two of the most powerful forces in manufacturing and now the broader supply chain are â€Å"Lean† and â€Å"Six Sigma. Traditionally, many companies have adopted one or the other as their primary approach to operational improvement, or in some cases used both but as fairly independent tools. Increasingly, however, companies are seeing the benefit of combining the two techniques into a more integrated strategy that uses the best of each approach, which can be highly complementary. Many believe this â€Å"Lean Six Sigma† strategy is the best w ay to improve overall supply chain results and tackle process improvement more holistically. Lean, the name given to the Toyota Production System in the book The Machine that Changed the World, has traditionally been associated with the elimination of waste in business processes. Lean was originally focused on improvement on the factory floor, but has since been used in some cases to power broader supply chain improvements. European retailer Tesco, for example, used Lean principles to engineer improved store replenishment processes. Six Sigma is a quality improvement methodology that in general seeks to reduce process and results variation. Originally focused on improving the quality of manufactured components, the approach has also been expanded for use in improving almost any business process. Drug wholesale McKesson, for example, has used Six Sigma to improve a variety of supply chain processes, such as inbound trailer cycle times and pick face replenishment efficiency. 2. 1 Adding Value With SCOR Developed by the not-for-profit Supply-Chain Council, SCOR (Supply Chain Operations Reference) is a model that links process elements, metrics, best practices and features associated with supply chain execution. It helps to identify and quantify critical opportunities for improvements not only within the supply chains of a single company but also between multiple trading partners. It describes a continuum of processes: Plan, Source, Make, Deliver and Return. SCOR works best with companies that have an enterprise information platform that can carefully track this continuum through KPIs and scorecards or at least provide the data so that a qualified enterprise platform can acquire the data, analyze it and surface it to the people who need to see it. Although rooted in the supply chain realm, SCOR adherents see a role for the methodology as the gatekeeper – identifying the projects most likely to render ROI using SCOR, Lean or Six Sigma. There is already a natural link between Lean and Six Sigma at the program and project execution level. Six Sigma has the project tracking and financial accountability elements. Lean helps â€Å"surface the rocks† that become ideal targets for Six Sigma. The two combined provide a powerful results-driven program. The opportunities for Six Sigma and Lean application in SCM are numerous. Start with Lean and look for opportunities to get the dock-to-stock (time from material unloaded on dock to put away on shelf) cycle times down (this impacts availability rates to the production floor). Take a walk through the warehouse and if you see lots of boxes and pallets sitting around, you know you have opportunities! Once your cycle times are lower, then take a look at defect rates in the other areas of standard warehouse metrics. Let me know if you have some questions I have extensive experience in this one. 3. 0 Bringing Lean and Six Sigma Together Industrial giant Honeywell was among the first to recognize the power of combining Lean and Six Sigma disciplines. When Honeywell acquired Allied Signal in the late 1990s, under the leadership of legendary CEO Larry Bossidy, it created a mechanism for combining Lean and Six Sigma that it called â€Å"Six Sigma Plus. The company hoped to improve processes and results by using Lean to streamline processes and eliminate waste, then improve the consistency and reliability of those processes using Six Sigma. 3M is another pioneer in the application of Lean Six Sigma. When Jim McNerney took over as CEO of the company in 2001, the former GE executive quickly helped drive a Lean Six Sigma program throughout the company. â€Å"Lean Six Sigma always starts by defining value from the supply chain from the eyes of the customer,† says Paul Husby, a former VP of Supply Chain Services at 3M. Every business has either an explicit or implicit strategy and needs the operational supply chain to provide specific performance to support that strategy. Lean Six Sigma has a primary goal of significantly improving operational excellence, and should also greatly improve a companys competitive advantage with key customers and markets. † There are several principles that drive the strategy of bringing Lean and Six Sigma together: Lean cannot bring a process under statistical control Six Sigma alone cannot dramatically improve process speed or reduce invested capital Both enable the reduction of the cost of complexity, but in complementary ways In the past, Lean and Six Sigma have at times been viewed almost as rival methodologies, with some companies choosing one or the other as their primary improvement vehicle. With Lean Six Sigma, that f alse conflict can be formally removed, and companies can benefit from the best of both approaches. The two methodologies can interact and reinforce one another, and there is much evidence that total improvements in a process is larger if Lean and Six Sigma are implemented together. The opportunity is to reduce the variability in value-adding processes,† said SCDigest editor Dan Gilmore. â€Å"Lean should eliminate the non-value added process steps, and then Six Sigma can be applied to tighten up the execution of those processes. † So, from a Lean perspective, what Six Sigma adds is the ability to reduce process variability. From a Six Sigma perspective, what Lean adds is often greater process and cycle time velocity, as well as lower operating costs. It can also be said that Lean focuses on reducing Time variability, while Six Sigma focuses on reducing Process variability. Lean tends to generate more â€Å"Quick Fix† solutions, while Six Sigma takes a more â€Å"Root Cause† approach. It is also possible to think of applying Six Sigma principles at the â€Å"pursue perfection† stage of one popular six-step Lean model. Some companies are taking the concept even further, adding in Eli Goldratt’s Theory of Constraints (TOC) methodology as another tool, usually front-ending TOC before both Lean and Six Sigma. This three-way combination is sometimes referred to as TLS (TOC, Lean, Six Sigma). Companies that have focused on either Lean or Six Sigma as a primary strategy to the exclusion of the other, or that use both but as very separate tools, should consider whether there is opportunity to deliver better results from a Lean Six Sigma strategy. 4. 0 THINGS NEED TO KEEP IN MIND 4. 1 Is our data integrated, clean and in one location? Data sitting in silos, incompatible or duplicative data, and – most importantly – data that requires the intervention of the IT department to retrieve will make any business improvement program much more costly than necessary. A robust business intelligence solution is a must. 4. 2 Can I quickly see what is working in quality programs? If you would like to report to Wall Street analysts that a Six Sigma initiative saved $1 million on a new product, do you need to deploy a portion of your staff to dig through information, or is that information readily available on a dashboard? More importantly, is the information up to date? Can a portal tell at a moment’s glance what the quality programs are doing for the company’s bottom line and how they compare to one another? 4. Can my staff visualize the results of quality projects? Six Sigma Black Belts parachute into departments they don’t work in. Being able to show how a process could be improved helps win over reluctant team members to the results of a project. An easily integrated visualization application is an important component. 4. 4 Do I have an application powerful enough to analyze and (especially when using Lean) forecast? Can my st aff – without any programming skills – create â€Å"what if† scenarios that will help identify the best methodology for improving quality and performance? Using SCOR to align corporate goals and develop and execute a plan for improvement targets, and then employing Lean or Six Sigma or both to the prioritized projects, is a way forward for companies that recognize that survival is truly the provenance of the fittest. Just having these methodologies under the roof isn’t enough. They need to be converged and integrated methodologies working off the same platform. A focus on integration will provide the organization with optimized yields and improvements. The trend toward converging methodologies is really about change management and its logical extension to platform status. After all, change is not a one-time event or even a series of one-time events but must be a culturally ingrained part of the organization’s DNA. Organizations want to embrace change, but doing so requires an integrated approach to using methodologies and a strong enterprisewide platform that goes beyond business intelligence to include data integration and analytics.

The Blood Clotting Cascade Biology Essay von Willebrand Factor is a large multimeric clotting protein which plays a significant role in the process of blood coagulation. It is mainly secreted by the vascular endothelial cells and also by megakaryocytes in the bone marrow. The vWF performs two important functions in the process of blood coagulation, and thats why it is very important. First, it is responsible in bringing together the elements to form the primary hemostatic plug. It serves as an anchor for platelets at the site of injury in the blood vessel. Second, it acts as a protective chaperone for Factor VIII, to avoid lysis by proteolytic agents in the blood. The Factor VIII also released by vWF at the site of injury, whereby it brings about the completion of the Intrinsic Pathway of blood coagulation, and seals the site of injury with Fibrin. von Willebrand Disease (vWD) is the most common inherited bleeding disorder in human beings the world over. Although, mutations in the vWF gene are responsible for the type of vWD in a patient, the transmission of vWD to the next generation is not solely linked to the vWF gene, but involves linkages with other genes such as the ABO blood type genes. The gene that encodes von Willebrand Factor is present on the short chromatid of chromosome 12, and is 178kb long with 52 exons or coding sites. Most of the exons are small, some as small as 41 base pairs (Schneppenheim, 2011). Exon 28, which has 1379 base pairs, is the largest. Mutations in the genes encoding vWF are primarily responsible for most vWD cases. Mutations can cause qualitative or quantitative deficiencies of vWF. Mutations in the vWF gene and the vWD that is attributed to the mutations are shown in Figure 1. These subunits dimerize (into 500 kilo Daltons subunits) and then multimerize into clusters greater than 10 mega Daltons in weight (Sadler JE, 2006). The vWF performs two functions serve as an anchor for binding platelets to the site of injury and bind to and stabilize Factor VIII from degradation by proteases in the blood and presenting it only at the site of injury. A vWF monomer has a repeated domain structure S D1 D2 D D3 A1 A2 A3 D4 B1 B2 B3 C1 C2 CK (Figure 1). The monomer is 2813 amino acids long. At the N-terminal is the 22 amino acid long signal peptide. Domains D and D3 are specific to Factor VIII binding. Platelets bind to vWF at its A1domain with their Glycoprotein (GP)-1b surface receptors. The A3 domain is specific to collagen, predominantly type III (J. Siekmann, 1998). Thus, domains A1 and A3 are necessary and must be fully functional to form the primary hemostatic plug in the process of coagulation. Figure 2: Domains of vWF protein, (U.S. Department of Health and Human Services, 2007) Overview of the clotting cascade A brief overview of the blood clotting cascade is necessary to understand the function of vWF in the process of clotting. The process of blood coagulation involves platelets and clotting proteins. At the site of injury in a blood vessel, the subendothelial collagen (types I and III) in the extracellular matrix of the blood vessel is exposed to blood. vWF that is present in the blood binds to the exposed collagen with its A3 domain. The flow of blood causes the multimers of the anchored vWF to unfold and expose the sites of platelet binding on the A1 domain (Figure 2). The platelets bind to this domain with their Gp-Ib receptor proteins present on the platelet cell surface. The binding of platelets to vWF activates them and a chemical messenger Thromboxane A2 is released by the platelets. Thromboxane A2 at the site of injury attracts more platelets in the blood, and aids in platelet aggregation. Platelets flowing in the blood stream bind to the activated platelets with a surface prot ein Gp IIb/IIIa. Fibrinogen (Factor I) is present in between the GP-IIb/IIIa receptors of two platelets. Thus, a primary hemostatic plug, though weak in strength, is formed. Figure 3: Sequence of events of blood coagulation due to vWF (U.S. Department of Health and Human Services, 2007) The unfolding of the multimers of vWF also releases Factor VIII at the site of injury. In the Intrinsic pathway of coagulation, Factor VIII is essential in catalyzing the conversion of Factor IX to Factor X, and eventually, prothrombin is catalyzed to from thrombin. Thrombin catalyzes the conversion of Fibrinogen (Factor I) into Fibrin. The fibrin forms a thick proteinaceous mesh, which seals the loss of blood from the blood vessel, this completing the process of hemostasis. Tissue repair and wound healing ensues. von Willebrand Disease von Willebrand Disease (vWD) is a deficiency of von Willebrand Factor. Based on the quantitative and qualitative deficiency, it is classified into subtypes. Combinations of assays are done to detect vWF levels in human plasma. Results of these tests report vWF levels in International Units per deciliter (IU/dL). The plasma concentration of vWF in healthy individuals is reported to be at around 10Â µg/mL (Mannucci, 1998), and the corresponding IU measurement is 100 IU/dL. The classification of the vWD types is based on the criteria developed by the vWF Subcommittee of the International Society of Thrombosis and Haemostasis at Carrboro, North Carolina, USA in 1994. Annual meetings are held by ISTH to review diagnosis and management guidelines for vWD by experts all over the world. The standard guidelines for the diagnosis and treatment of vWD in the USA is based on the vWF Report by the National Heart, Lung and Blood Institute, National Institutes of Health, U.S. Department of Health and Human Services, which was released in 2007 by the expert panel on vWF, chaired by Dr. William L. Nichols, Jr., M.D. The ISTH holds annual meetings all over the world to discuss updates on vWD. The first vWD classification by the ISTH in 1994 was based on information about mutations on the vWF gene. However, because it was appropriate to only a small population of the human race, it was overruled in 2006 and was replaced by the new method based on response to treatment with DDAVP or other blood based therapeutics. vWD is classified based on qualitative and quantitative deficiencies. Partial quantitative deficiency is type 1 vWD and total quantitative deficiency is type 3. Qualitative deficiency is type 2, and is subdivided into types 2A, 2B, 2M and 2N based on the functions of the vWF which are affected. Quantitative deficiencies of vWF are discussed first types 1 and 3 vWD. The main laboratory tests to analyze vWF in patient samples are vWF:Antigen activity (vWF:Ag), Factor VIII: Coagulation activity (FVIII:C) and vWF: Ristocetin Cofactor activity (vWF:RCo). Type 1 vWD A patient with partial quantitative deficiency of vWD is diagnosed as type 1 vWD. The level of vWF in the plasma, though low, can still carry out the formation of the primary hemostatic plug, and also protect Factor VIII. In most type 1 vWD cases, Factor VIII levels are very mildly affected. It is hard to accurately diagnose type 1 vWD because, the vWF levels also depend on the ABO blood grouping. The average vWF level in healthy individuals with blood type O is about 75 IU/dL. It is reasonable to classify the condition of a patient with less than 20 IU/dL vWF level as type 1 vWD because this indicates a probable hereditary mutation. The vWF:Ag and vWF:RCo tests show similar reductions in vWF activity for type 1 vWD patients compared to the reference plasma by ISTH (U.S. Department of Health and Human Services, 2007). Type 3 vWD When the vWF activity of a plasma sample is less than 10 IU/dL, it is classified as type 3 vWD.84-86 Major mutations such as frameshifts, large deletions, splice-site mutations, and missense mutations can be causes for type 3 vWD (U.S. Department of Health and Human Services, 2007). Sometimes, clearance of vWF from the blood stream due to autoimmune disorders can decrease vWF quantity in the blood to type 3 levels of vWD. This is also one of the causes of of Acquired von Willebrand Syndrome AVWS. Type 2 vWD Type 2 vWD is a qualitative deficiency of vWF, where, although the vWF may be produced in normal quantities, fails to perform its tasks effectively. Based on the defect in the von Willebrand Factor, it is mainly classified into types 2A, 2B, 2M and 2N. In type 2A vWD, the vWF platelet binding activity is decreased due to the absence or deficiency of high molecular weight multimers of vWF. There is a sharp fall in the vWF:RCo activity, but not much decrease in vWF:Ag and FVIII:C activity. This is because the vWF is still able to bind to Factor VIII. (Ruggeri ZM, 1980) The high molecular weight multimers are either degraded by proteolytic enzymes in the blood or have not been produced due to mutations in the exons of the vWF gene that code for the A2 and/or the D3 domain. (Schneppenheim R, 2001), (Sutherland JJ, 2004) . Type 2B VWD is characterized by an abnormal increase in the vWF-platelet binding affinity, which leads to depletion of large, functional VWF multimers, and also a fall in platelet numbers (Zimmerman TS, 1986) The platelets circulating in the blood stream are blocked with the mutant vWF, due to which, there is a great difficulty in the formation of the primary hemostatic plug. Thus, thrombocytopenia ensues, along with increased Ristocetin Induced Platelet Aggregation (RIPA) even at low concentrations of Ristocetin. Mutations in the A1 domain are responsible for type 2B vWD (Huizinga EG, 2002). In type 2M vWD the vWF platelet binding activity is reduced. But unlike the type 2A vWD, there is no decrease in the quantities of high molecular weight multimers. This phenomenon is only due to a decreased affinity to the Gp-1b receptors on the platelets (Ginsburg D, 1993), (Schneppenheim R, 2001), (Meyer D, 2001), (Rabinowitz I, 1992) (Mazurier C, 2001). The vWF:Ag, vWF:RCo and FVIII:C activities in types 2A and 2M vWD are similar. They can only be diagnosed based on high resolution gel electrophoresis images. (Meyer D, 2001). Another phenotype of type 2M vWD is the failure of vWF to bind to collagen in the extracellular matrix of the vascular sub-endothelium. vWD type 2N is due to the absence of vWF-Factor VIII binding. This is due to mutations in the D and D3 domains of the vWF protein (Ginsburg D, 1993), (Mazurier C, 2001). The laboratory tests for this type of vWD indicate normal levels for vWF:Ag and vWF:RCo tests, but the FVIII:C activity is only about 10% of normal levels. Acquired von Willebrand Syndrome Acquired von Willebrand Syndrome (AVWS) is type of vWD which is not genetically linked. There are three mechanisms by which it is manifested: Autoimmune reactions against vWF, Increased proteolysis of vWF by a protease ADAMTS13 (A Disintegrin And Metalloproteinase with a ThromboSpondin type 1 motif, member 13), or abnormal increase in the binding affinity of vWF to platelets or other cell surface receptors (U.S. Department of Health and Human Services, 2007). Diagnosis of vWD The diagnosis of von Willebrand Disease and its sub-type is made based on an initial review of previous health conditions and familial history of bleeding disorders, which is done in the clinic, and then obtaining plasma samples of the patient for laboratory tests.

Macbeth Character Analysis Essay

In this essay I will write about the character of Macbeth and what my first impressions are of him, such as his bravery, strength, determination and courage. I will also write about how he is introduced, his reaction to the encounter with the weird sisters, his attitude towards Banquo, Duncan and Malcolm. Also, his relationship with his wife, Lady Macbeth. My very first impression of Macbeth was that he was was brave as the captain said, â€Å"For brave Macbeth – well he deserves that name†¦Ã¢â‚¬ . Also, the captain implied that he is very strong and a great fighter, â€Å"†¦ With his brandished steel, which smoked with bloody execution, like valour’s minion carved out his passage, till he faced the slave†¦Ã¢â‚¬ . This means that he stands with his sword covered in blood which shows he has killed a lot of people and valour’s minion means bravery’s favourite. The captain also says how merciless Macbeth is as he kills the traitor Macdonwald, â€Å"†¦which never shook hands, nor bade farewell to him, till he unseamed him from the nave to the chops, and fixed his head upon our battlements†. This quote shows how merciless Macbeth is as it states that he ‘carved’ his way through the army, and how he killed Macdonwald even after bring tired and bruised without even speaking to him. He even paraded his head on the battlefield to show everybody what he had done. Our first impressions of Macbeth are that he is brave, merciless and a great fighter. The three witches soon appeared and told their predictions; â€Å"All hail Macbeth, hail to thee, Thane of Glamis!† â€Å"All hail Macbeth, hail to thee, Thane of Cawdor!† â€Å"All hail Macbeth, hail to thee, that shalt be King hereafter!†. After Macbeth heard the wierd sisters he was too shocked to speak as Banquo said, â€Å"Good sir, why do you start, and seem to fear things that do sound so fair†¦Ã¢â‚¬  He finally answered back by asking them to tell more and in more detail. He started to question the three witches and himself as he was confused; â€Å"†¦I know I am Thane of Glamis, but how of Cawdor? The thane of Cawdor lives†¦Ã¢â‚¬  . He was trying to figure out what this all meant and how these creatures got this information. He again tried to question them but the suddenly vanished into thin air, â€Å"The earth hath bubbles, as the water has, and these are of them.†. He asked Banquo if this was real or if they had eaten some kind of hallucinogens or drugs, â€Å"Were such things here as we do speak about? Or have we eaten on the insane root†¦Ã¢â‚¬  He then started thinking about being Thane of Cawdor and whether or not this could be true. The two messengers came, nearly straight after the predictions by the three witches, and told Macbeth that he was given the title Thane of Cawdor. Macbeth was in shock and Banquo said that evil was at work, Macbeth asked the messengers why he was the new Thane even though the old Thane still lived. He thanked the messengers and said to himself that more was to come as this is shown in the quote, â€Å"Glamis, and Thane of Cawdor. The greatest is behind.†. This means that the greatest prize is still to come and this is referring to him being King. His first thoughts of the three witches after they appeared and told their predictions were that they were some kind of supernatural beings and so Macbeth tried to find out what they meant, where they received this information and why they were telling him this. He was first thinking this was some kind of hoax but after the messengers came and said that the King had named him the Thane of Cawdor, he began to consider what it will mean if the third prediction is to come true. Banquo is Macbeth’s best friend and he trusts him with his life and vice versa. He respects Banquo and he uses his advice most of the time. They are partners in war and in friendship and do almost everything together. He likes Banquo but he sometimes has different thoughts than him as this quote states, â€Å"†¦But ’tis strange: And oftentimes, to win us to our harm, the instruments of darkness tell us truths†¦in deepest consequences.† This quote means that the recent occurrences are unnatural and the forces of evil encourage them to do things, which are misleading, and there could be some consequences later. Duncan is the King of Scotland and Macbeth had given him the most respect he has ever given anyone. Macbeth was loyal to the King and fought for him and the country of Scotland. Macbeth thinks that Duncan is a good King and uses his power to good strength and also the fact that he has a good judgement, this quote shows this, â€Å"Strong both against the deed; then†¦besides Duncan hath borne his faculties so meek, hath been so clear in his great office†¦Ã¢â‚¬  This means that Macbeth thinks King Duncan is a good king who is not corrupt. Macbeth does not like Malcolm even though he is related to him, Malcolm is the heir to the throne and that is a bit of a pain in the neck. Macbeth wants to be king but cannot as Malcolm is the heir so, as you would imagine he is an obstacle in Macbeth’s ambition and target, this quote shows this. â€Å"The Prince of Cumberland-that is a step†¦Ã¢â‚¬  Lady Macbeth is Macbeth’s wife and Macbeth loves her very much and so he sends her a letter telling her that he has become the Thane of Cawdor, â€Å"They met me in a day of success†¦they have more in them than mortal knowledge†¦came missives from the King, who hailed me ‘Thane of Cawdor’†¦ â€Å" Lady Macbeth is the kind of person that gets what she wants and I don’t think that Macbeth likes this that much. She persuades him to murder Duncan nearly straight after she recieves his letter. Macbeth likes Lady Macbeth but he doesn’t like the way she gets her way but he still goes with the flow, â€Å"Was the hope drunk wherein you dressed yourself†¦and live a coward in thine own esteem†¦each corporal agent to this terrible feat. Away, and mock the time with fairest show†¦ â€Å" Here, Lady Macbeth uses reverse psychology by accusing him of cowardice and a lack of love or her. Overall the first impressions we get of Macbeth are that he is brave and a good fighter and that he is loyal to his country and the King. His first impressions of the three witches were the same as anybody else, surprise and that the witches are some strange people or that they are seeing things. He likes Banquo and respects him and he is very loyal to Duncan who is like a father figure to him. He doesn’t like Malcolm that much as he is an obstacle but he is still related to him. His relationship with Lady Macbeth is very good but you get the impression that she is very much in control of their relationship.

He set it in 1912 because he knew what was to come, this way he had an insight to what was going to happen, so he could show the ignorance of the upper sector. Mr. Birling should understand the common people better than he does, especially as he came from a working class background. It would seem that he does not appreciate the working class anymore, as he would not even increase the wages of his workers. â€Å"They were averaging about twenty-two and six which was neither more nor less then is paid generally in our industry. They wanted the rates raised so that they could average about twenty-five shillings a week. † It is also evident that he runs his life by looking after himself first, as he tells Gerald and Eric at the beginning of the play â€Å"And as you were saying, dad, a man has to look after himself. † Mrs. Birling should have some understanding of Eva Smith because she is a governor of a charitable organisation to help women in desperate situations. But her ignorance gets in the way of her work, she doesn’t even think there is a possibility of there being more than one Birling family, so she dismisses Eva Smith straight away. â€Å"Yes I think it was simply a piece of gross impertenance – quite deliberate – and natrally that was one of the things that pedjudiced me against her. † Mrs. Birling’s ignorance is also apparent when she is saying what should happen to the young man who impregnated Eva Smith. Sheila had to learn, not to use status as an extended tool for jealousy. After Sheila has been told about her involvement in Eva Smith’s death she seems genuinly sorry for what she has done â€Å"And I know I’m to blame – and I’m desperately sorry† As well as what Sheila says, the stage directions like â€Å"distressed† are used to show how she is feeling. Her youth also opens her to the exposure of Inspector Goole â€Å"We are most impressionable on the young† this also makes us realise how Inspector Goole â€Å"breaks down the barriers†. When you are â€Å"far gone†, or as you would more commonly say it now, drunk, you are quite likely to get yourself into some sort of trouble as Eric learns. Unlike his parents and Sheila, Eric realises that what he did was wrong at the time and tried to make amends for it by giving Eva Smith money. â€Å"I insisted on giving her enough money to keep her going† At first when Eva Smith left him he didn’t change his ways, you can see this when he â€Å"helps himself to the decanter†. After the Inspector has gone through the events it looks like Eric has finally come to grips with what he has done and will make some sort of effort to change now, because of the imppression left on him by the Inspector. The message Inspector Goole is trying to present is that, whatever you do, you will eventually be judged for it. â€Å"We are members of one body. And I tell you that the time will soon come when, if men will not learn that lesson, then they will be taught it in fire, blood and anguish. † Inspector Goole also delivers the message that everyone must look after everyone else, and almost exactly contradicts Mr. Birling’s teaching of â€Å"every man has to look out for himself† or else the world will turn into a cold, miserable place, full of hatred and selfishness.

It is a day in the history of our country in which everyone remembers where they were and what they were doing when they heard the news that two planes had flown into the World Trade Center in New York City, taking out both of the Twin Towers. All in all there were close to 3,000 people who lost their lives on that fateful day between New York City, Washington DC, and Pennsylvania. The attacks on the†¦show more content†¦There has been a substantial change since September 11, 2001 in how law enforcement agencies attempt to prevent terrorism as well as how they respond to terrorism, and the role of intelligence agencies in preventing terrorism. Intelligence agencies are â€Å"permitted to college a large amount of foreign intelligence that is of interest to law enforcement† (Permanent Select Committee on Intelligence). That which qualifies as interest to law enforcement, and is in the realm of what intelligence is to be looking for is defined as â€Å"Information concerning†¦activities directed against the United States, international terrorist and international narcotics activities, and hostile activities directed against the United States by foreign powers, organizations, persons or their agents† (Permanent Select Committee on Intelligence). Intelligence agencies are also collecting information for â€Å"political and military intelligence for policy makers† (Permanent Select Committee on Intelligence. Intelligence focuses on many types of information, and â€Å"there are few rules governing intelligence gathering†, (Permanent Select Committee on Intelligence). Within the United States we have many different intelligence agencies, most of which existed during September 11. One of the most well known intelligence agencies is The Central Intelligence Agency, their
